This information includes, but is not limited to, name, address, telephone number, mobile number and/or email address, medical history, medications history and the name of the treating doctor.

Once collected, Medata will store Your Information for a reasonable period of time with respect to the limitations of space and other administrative functions in the course of Medata business.
